Disinfection without cleaning first is wasted
Disinfectants work through concentration and dwell time, and blood, saliva and protein residues consume the active substance — that is the protein error, which affects preparations based on quaternary ammonium compounds particularly. Those compounds are also incompatible with anionic surfactants: if soap is used to pre-clean and is not rinsed off, the two cancel each other out. And every surface has to stay visibly damp for the whole dwell time; anything wiped dry immediately has been dampened, not disinfected. That is why a usable plan states the concentration, dwell time and efficacy range for every product, and our staff work to that rather than by eye.
The separation of clean and unclean applies to the mop pad too
In the reprocessing room the instrument travels in one direction: intake and pre-cleaning on the unclean side, inspection, packaging and sterilisation on the clean side. Cleaning that takes the same pad first through the sterilisation area and then through the intake side carries back exactly the contamination the room layout was built against — and the same applies to the route from the WC into the treatment room. We work with a colour code for each zone, single-use pads and a fixed sequence from clean towards unclean, and we do not clean the clean side while unpacked sterile goods are lying there. Reprocessing, release and their documentation remain with the practice team.
Alcohol is the fastest product, not the strongest — and it does damage
Rapid alcoholic disinfection works reliably against enveloped viruses such as hepatitis B and C or HIV, but only to a limited extent against non-enveloped ones such as noroviruses or adenoviruses; there a preparation with a correspondingly extended efficacy range is needed. At the same time alcohol is the commonest reason for ruined practice surfaces: synthetic leather covers on treatment chairs go brittle and split along the seams, polycarbonate and acrylic parts develop stress cracks, displays lose their coating. Chloride-bearing products attack stainless steel with pitting. We select for each surface on both grounds — efficacy range and the equipment manufacturer's material approval — because otherwise the next item of expenditure is the upholstery.
