In practice it means that the bed frame, the bedside cabinet, the hand control, the drip stand and the door handle in a patient room are worked more often and with disinfection than the floor of the same room — the floor is of less hygienic relevance than the surface that is touched a hundred times a day. A schedule of services that names only square metres, room type and frequency therefore passes the hygiene plan by; it becomes usable only once the contact surfaces close to the patient appear in it as an item of their own with a frequency of their own. Terminal disinfection is a procedure, not a product
In terminal disinfection the method decides: from clean to unclean, from top to bottom, one pad per surface or zone with no re-dipping into the solution, sufficient wetting and the full contact time before the room is occupied again. That includes the parts that are readily overlooked: curtains and privacy screens into the laundry cycle, mattress and cover to the manufacturer's instructions, the floor drain with its water seal, high-level surfaces and ventilation outlets, the remote control and the telephone. With the organism it becomes specific: against Clostridioides difficile sporicidal actives are needed, against noroviruses agents listed as virucidal — both load materials more heavily than the routine cleaner, so the manufacturer's approvals for synthetic leather, displays and stainless steel belong settled beforehand. The area is released after a check, not by the clock, and the check is documented.
Building work in an occupied hospital is a hygiene subject, not just a dust subject
Opening ceilings, ducts and service walls releases spores that occur in the ambient air anyway but become relevant where patients are immunosuppressed — mould spores from old layered construction are among them.
